We accept VSP and VSP-affiliated plans such as Guardian, Metlife, Traditional Medicare or Medicare PPO (not HMO), All Kids, active BCBS of Alabama** (in-state and  excluding EPO and  HPN plans). 
**BCBS PCP designation and referral may be required by certain plans for eligibility of services.
 BCBS Medicare Advantage, Viva UAB, and Viva Medicare, and PEEHIP Medicare
*All other plans and Secondary plans may be filed by the patient for reimbursement if eligible.
